Richiesta del certificato di morte thailandese

L’Ambasciata thailandese può rilasciare un certificato di morte thailandese per cittadini thailandesi o per cittadini stranieri che abbiano ottenuto il permesso di residenza in Thailandia ai sensi della Legge sull’Immigrazione B.E. 2522 (1979).
La richiesta può essere presentata dal genitore, coniuge, discendente della persona deceduta, oppure da un delegato di uno di essi.
Il certificato di morte thailandese potrà essere utilizzato per la registrazione ufficiale del decesso in Thailandia.

Documenti richiesti

1. Modulo di richiesta per la denuncia di morte (scarica il modulo)
2. Certificato di morte originale rilasciato dall’Ufficio dello Stato Civile del Comune, legalizzato dalla Prefettura
3. Copia del certificato di morte rilasciato dall’ospedale, contenente chiaramente la causa del decesso, la data e l’ora della morte, il periodo di ricovero, nonché il nome e l’indirizzo del luogo del decesso.
4. Copia del documento di identità del defunto, ad esempio carta d’identità o passaporto.
5. Copia del certificato di residenza (Tabien Baan) in Thailandia e in Italia del defunto.
6. Copia del documento di identità del richiedente, ad esempio carta d’identità o passaporto.

Thai Death Certificate

The Royal Thai Embassy in Rome can issue a Thai death certificate for deceased persons who are Thai nationals or foreign nationals who have been granted permanent residence in Thailand in accordance with the Immigration Act B.E. 2522 (1979).

The application may be submitted by the parents, spouse, or descendants of the deceased person, or by an authorized representative of any of these individuals.
The Thai death certificate can be used for the official registration of death in Thailand with the relevant Thai authorities.

Required Documents

1. Death registration form (Downloadable form)
2. The original Italian death certificate issued by the local Comune (Municipality), duly legalized by the Prefettura (Office of Legalization and Citizenship).
3. A copy of the hospital declaration, clearly stating the cause of death, date and time of death, duration of hospitalization, and the name and address of the place of death.
4. A copy of the deceased’s identification document, such as a Thai national ID card or passport.
5. A copy of the house registration (Tabien Baan) both in Thailand and Italy of the deceased.
6. A copy of the applicant’s identification document, such as a Thai national ID card or passport.
